Prescriptions

Requests for prescription refills will only be handled during the following office hours:
Monday through Thursday, 9:00 am to 5:00 pm and Friday, 9:00 am to 12:00 pm. Requests taken after 12:00 pm
on Friday will not be processed until Monday. All refills require 24-hour notice.  Be prepared to provide your
pharmacy name and telephone number in order for requests to be processed. Requests for narcotic
medications will not be taken after hours or on weekends. Please review your personal prescription
drug program carefully as new guidelines require special authorization for certain drugs, and new
formularies may eliminate the specific prescription drug you are now taking. These factors could play
a role in the timeliness of your prescription refill. 

Release of Medical Information

Your medical records are kept strictly confidential. Before we can release any
records, you must sign an authorization form. A copying and retrieval fee will
be charged for the preparation of medical records and x-rays for attorneys and
insurance companies.  Our office requires 24-hour notice to prepare copies
of medical records or x-rays. 

Diagnostic testing
If you are scheduled for a diagnostic test (MRI, CT scan, nuclear medicine scan or arthrogram), you will
need to take copies of your x-rays with you to the testing facility. When appropriate, please request your
x-rays before leaving the office. When necessary, please make arrangement with the providing facility to
pick up the written report and the actual films and bring them with you to your follow-up appointment. 

Disability Forms
Our office will complete your disability insurance claim form for a minimal charge. The fee for each form
must be paid in advance or at the time you receive your completed form. If you have asked us to mail
your form directly to your insurance company, you will be required to pay the fee when you drop the
form off at our office.
